Your medical records

When you apply for or receive care, medical records are always kept. The main purpose of medical records is to ensure your care is documented by the staff who provide you with care.

Under the Patient Data Act, patient records must include the following:

  • Personal details
  • Information about why you have received care.
  • Details of the diagnosis you have been given and the examinations and treatment you have undergone.

Read your medical records

In principle, you are always entitled to read your own medical records after the doctor, or another person who is responsible for the medical records, has conducted a review. To gain access to your medical records after you have been a patient at Skaraborg Hospital, you can print out and complete the form:

You should then send the form to the hospital by post. The address can be found on the form. 

Since June 1, 2016, you also have access to certain information in the medical records via 1177 Care Guide e-services by logging on at 1177.se. You will find details of your medical records under the heading Journalen.
